Fear has prevented many qualified patients from enjoying the benefits of LASIK vision correction. Surgeons have long recognized that complications, although extremely rare, involve creation of the corneal flap. While microkeratomes have improved their already excellent safety profile over the past 10 years and laser-based microkeratomes (Intralase™) have become more effective, the corneal flap remains the achilles heel of LASIK.
Bladeless LASIK, or Epi-LASIK requires no flap, no microkeratome, and no Intralase™. It is the refinement of a now archaic procedure called PRK. Epi-LASIK cleaves the surface epithelium and leaves an immaculate surface on which to precisely reshape the cornea.
Because there is no corneal flap Epi-LASIK essentially eliminates all potential flap complications. Without a flap to reposition, all wavefront errors induced by flap creation, inherently flawed re-alignment, and flap wrinkles are eliminated. Because the cornea is laser reshaped from a perfect optical surface, the visual quality and contrast sensitivity are superior. Less of the corneal anatomy is compromised reducing the need for enhancement surgery. Finally, because the maximum amount of untouched cornea is preserved, the structural integrity is maintained and long term risks from trauma and the chance of ectasia are dramatically reduced.
Surgeon's knew that PRK had some of these properties but they knew it required the use of toxic chemicals like alcohol and the trauma of a rotating brush to reach the perfect optical surface shown to our left. This was painful, required months of topical steroids and often generated corneal scarring, regression, and distorted vision.
